New To AGA/Need 2016 Paperwork
We must have an up-to-date AGA Student Information and Release Form on file for every child participating in a class or event at AGA PRIOR to entering the gym. This form is good for all events for the entire year, so if you haven’t yet filled out the 2016 form, please do so after enrolling.
Forms
- AGA Student Information and Release Form – We have consolidated our paperwork into one form for the entire year. If your child is enrolling in a class, or participating in a camp we need to have a AGA Student Information and Release Form filled out and on file before your child enters the gym. Once the paperwork is filled out and on file, it will be good for all classes for the entire year. To make things easier for parents with multiple children, you can fill out one registration form and individual signed waivers for up to 3 of your kids. Enrollment in classes, camps or other events, should be completed PRIOR to filling out the Student Release Form as this form will NOT enroll/register your child for any class or event.
